Decorative Concrete over Plywood Subfloor

I am looking to install decorative concrete over a plywood subfloor in my house. I have been getting different opinions about whether or not it can be done. I am curious if anyone knows if the concrete can be poured over the plywood floor no more than 2 inches and have it not crack over time. The house was built in 1992 so it should be finished settling. I also need to know how to prepare the subfloor to prevent moisture and to reinforce it enough to handle the weight of the concrete (there is about a 3 foot crawl space under the house). Lastly, I want to know the recommended lightweight concrete to use and whether to use some type of reinforcement (i.e. hog mesh or lath diamond mesh) in the concrete. Any advice would be appreciated and if there are any online tutorials or books on this it would be great. Thanks.
