Is the width of an item the only factor that determines if it will fit throuth a door opening (couch in particular).

Depends if you have an unimpeded doorway. Often mobile homes have restricting passageways which may force delivery people to "stand it up on end" in order to make it through a doorway. If you live in a regular house, you really shouldn't worry about that aspect.
