Vinyl on concrete slab ?

What's the best way to prepare a concrete slab floor for the installation of perimeter bond
vinyl?

This is for a small bathroom on a slab floor that has until recently been leaking water
through the cold joint. (The moisture problem has been fixed with the installation of a
foundation drain outside and another drain inside under the slab.)


Considering the history of water infiltration ...

What's the best underlayment ... particle board, plywood, or perhaps marine grade
plywood?

Is plastic the preferred vapor barrier over concrete? or is tar paper a viable alternative ?

And lastly ... how is the underlayment attached to the slab ? glue? gravity?


Any help would be greatly appreciated,
