tiling over wall tile

i am looking to tile over the porcelain tile around the tub. there is a surround that i am going to replace but i want to tile the border area that the surround does not cover. there are 4x4's there now (porcelain) that i want to cover with 6x6 porcelain.
-can i go right over the existing?
-do i need to backerboard over existing?
-if not what should i use to remove the surround adhesive?
-am i nuts?
i want to recover the ceiling over the tub to so is there a way around tearing that oput first? i'm thinking of cutting a template to fit then tiling backerboard to fit that, tiling then installing but would that work? would i slide it in after the walls are done?
