Is it possible to tap into a 4 " cleanout for a sink. Is there a 4" pvc connection to use as cleanout and 2" sink drain pipe?

Anything is possible after all we did put a man on the moon.

I would use a WYE possibly a 4 x 4 x 2 and a 4" M x PVC adapter and a 4" F FTG

This way you still have the full sized clean out and on the branch y you can install a 2" 45 degree fitting and don't forget the vent.

Now you can place the plug back on the top of the Tee and thus have the capability of rodding the line if needed.

The reason you want to go wirth 45 deg and not 90's is less chance of a stoppage and much less noise and must better drainage
