I am hanging a new chandelier/pendent over the kitchen table. What is the industry standard for the the distance between the bottom of the lighting fixture and the top of the table?

I believe that there is no code as to the height of the table to the light,... So I would put it where you want it. If you are 5'4" or 6'6" there is a proper height that only you can judge.....

I prefer to have it just above head height because even though it will be over a table, some time - sooner or later - you will bump your head. And that may damage the chandelier as well as your head.
