I have metal gutters with some small rust stains on them but no holes. I want to paint the outside of the gutter a different color. How do I clean the rust off and do I/what type of primer should I use?

I clean off rust with one of those rotary brush attachments that go in your drill.

Then prime with Rustoleum metal primer and paint with latex or oil base paint.

I would be sure to check the inside of the gutter to see if the rust hasn't started there.

this could be more trouble than it's worth, check out a local seamless gutter installer and see what they would charge for a whole new gutter system usually it is fairly decent priced.
