A cement board should be put over the plywood to give a superior bond for the tiles. Thinset should be used under the cement board as a filler and then anchored with galvanized roofing nails or backerboard screws. DO NOT OMIT THIS STEP!!!
A floor needs to be flat for tile but not necessary to be level. If you want it level then pour the SLC over the cement board in the low spots. Make sure any gaps are sealed shut or that stuff will run into the wall cavity.
Set the tile then put the toilet in, use a flange extender or a double wax ring if height is an issue.
