EFFLORESCENCE

More than likely one of 2 things happened:

1) Tile was laid recently and too much water was used in the grout.

2) Grout was never sealed and over time the water on it caused this.

It's efflorescence - salts in the grout coming out.

Remove with an acid solution (hydrochloric) and wipe dry. It's the only way. If you try sanding it out you'll pit the grout and it actually looks worse.
