Beam Failure....

"crack in a lower portion of a glu-lam beam" It sounds like you either have a tensile failure or a horizontal shear failure in the beam. It is my opinion from the limited amount of information you have given that either the beam was undersized for the job, or there is a manufacturing defect in the beam. If you can identify the manufacturer, you may be able to get assistance from them.
What size is the beam? A 24 foot span sounds excessive for the amount of load it is holding.
How much sag is in the beam? Pull a string tight from one end to the other and measure the total sag in the middle.
I agree with tomh and Billy, don't let this go, you may wake up in the basement some night!
