Tile Fireplace

I would like to tile over my brick fireplace. Problems are 1) it's painted - I've heard this is a bond breaker for thinset. Is there some sort of bonding agent I can use or is it necessary to somehow remove the paint (ugh!)
2) The face of the fireplace is "bumpy" brick. The hearth is the smooth brick. Should I install a backerboard to even out the face or layer of thinset? I really am uneasy about trying to use the thinset to "level" it because I think it'll be too thick in some areas. Heard mention of "Ditra" - what is it?
3) Is there any "standard" size tile used for fireplaces (i.e. 8x8, 12x12 whatever)? Are there more problems with using larger tiles - what size "looks" right?
