Linda, the best way is to remove the window, if you can easily remove the stop moldings, and take it to a glass shop. If you can't remove the window easily, you will have to remove the putty that holds the glass in place on the outside of the window. Old putty can be really tough to remove and things like a putty knife and propane torch are some of the tools that you MAY have to use. If you do have to use the torch, you will melt paint that probably has leasd in it so wear an appropriate mask. It's pretty tough to tell you exactly how to do this because the age and condition of the putty makes every window different. The only rule to this is to not do more damage to the window. The glass will be actually held in place by triangular bits of metal that must be removed from the wood mutton bars or mullions for further replacement.

Once you have all the glass out and the opening cleaned up of old putty, you can measure the opening with any suitable measureing tool from a tape measure to a yard stick. Measure at a couple of places and record your measurements on a sketch. Take the sketch to the glass shop for new glass, glazier's points, putty and instructions on how to do the replacing.
