If it was me....: If your subfloor is level and sturdy I would get some resurfacing morter (Sacrete) and fix any bumps or holes that may be left and let dry. I would then place the pavers in place. Some pavers have a bump on them that will create a 1/8 to 1/4 gap for you (get this kind.) You can then fill the cracks with sand. I don't like the sand because you have to refill once in a while. I started using a very soupy morter that I pour out and work in with a broom. Once I get it worked in good I spray it with water and sweep some more. Let it sit for about a half hour then sweep again. Keep the morter that is on the pavers wet between sweeps until it is all cleaned up. This may be a wrong or crazy way of doing it but it has worked great for me and has lasted for 3 years so far with no maintenace.
