Sagging floor in 100 year-old-house

I'm thinking of buying a house - an older terraced property.

However, there is a sag in the floor on both storeys, just enough to be noticeable. It has apparently been the same for many years.

Also, in the basement/cellar, I see a big steel joist running across the width of the house between the supporting walls (clearly something that has been added by an owner, not the original builders). Otherwise the house seems free of subsidence - outer walls are straight and crack-free.

Should I proceed with an expensive survey to find out more or just back out now? Might this be a quaint but harmless feature?
