The only thing I can think of is that when they sanded it, the wood itself was exposed and dried, which is normal. The wood then had a crack initiation site, which propagated once you began to walk on it. If it's "large" (e.i. greater than 1/16", or large enough for filler) fill it in with a good wood filler, or good glue (e.g. PL400) and put saw dust on the top. Then urethane over the top and then only you should know it's there. Good luck.
