Obviously there is a moisture problem. Is it on concrete or wood? if there is a concrete slab, then the question is did you use a sheet of plastic as a vapor barrier? ..If wood.. then you probably have an appliance leak. Maybe an ice maker water line?
Either way .. you need to find the water leak.
