as a rule .. main floors should be 3/4" plywood.. a subfloor can be as much as 1/2" on top of that. No the nails should not be sunk in 1/4" you are correct. However they should be counter sunk in so none of the head will show and the surface is flush.
Also, ringshank nails should be used so they won't work themselves out later on.
