That would depend on the brand of the cooktop appliance not just the glass used on it. You can read about what information you need to get replacement parts for your home appliances in the 'Repair Parts' section of by site linked below.

In most cases (being that Corning got out of the appliance accessories business 20+{?} yeas ago) the best you could hope for is that a replacement in the newer black glass which would fit is available.

For that you'd likely have to see if you can have something custom made. Check with a local glass supplier or kitchen cabinet place to see if they have or can get a suitable replacement.
