Yes, the original K&T wiring was all black. They later went to black & white.
How to tap into the middle of a run of K&T wiring is going to be interesting. You will have to run it into an electrical box in two places so you can splice to it; two places to get some slack. I don't know where to find the special parts needed to do it to pass code; ask at an electrical supply house.
I worked under an old church a couple weeks back (built in 1910, I believe) and they had the neatest K&T wiring have seen. In a couple places where it went into conduit, three wires, they used three hole porcelain insulators that fit into a metal part that looked like a flat fan-shaper lawn sprinker nozzle. These were connected to a modern electrical box.
As to color, it makes no difference since it is ac. It would only be a concern if it terminated in an outlet. There could be a question of the insulation in the fan, but as long as it is not grounded, and as long as you don't touch the metal parts, there is no danger.
