Hey, I can relate to your current situation. My wife and I are looking to buy a house whose previous owners were heavy smokers for 10 years! I have contacted many general contractors and this is what they have told me:
-It is not enought to just get new carpet, you have to seal the cement under the carpet with Killz.
-The walls must be sealed with killz and then painted.
-The cabinets must be sealed with Killz.
-basically every part of the inside of the house, this includes seilings, must be sealed with killz.
-you have to have a certified airduct person go and clean out your ducts. They need to be certified and be able to get to the "Squirrel Cage". Apparently not everyone is licensed to go to the "Squirrel Cage" but those who can are able to get rid of the smell.

What is your advice. Do you think we should buy the house?
