Go to your local hardware store and buy a phone jack. You can get one that you can just screw onto your baseboard or one that you attach to the wall itself (it's like a switchplate).

There should be directions with it. Basically when you open it up, there are four "terminals" to attach the wires to. Each of them will also have a "U" shaped metal thingie (don't know what they're called) and sometimes 2 small round washer type thingies on each terminal. Strip the outside coating off of the phone wire if that's not already done. That will expose four colored wires...Red, Yellow, Green and Black. Strip about a half inch of the rubber coating off of these wires, and simply wrap it around the corresponding terminal. The terminals are marked with a B,Y,G and R...so it's pretty simple to do. Just wrap the exposed portion of the wire between the two washers on each terminal and tighten the screws (leave the U-shaped thing on there too).

It's a really simple process.
