exterior Sheathing application

My home was built in 1995. It has a black fiberboard with the letters OSB on it.
When this fiberboard gets wet it will crumble like sawdust. Here is how our walls were built:
1/8 inch coard on inside of home,
insulation between studs
1/2 inch black fiberboard on exterior
vynyle siding over fiberboard
Some of the fiberboards were not attached to studs and some had gaps on gableends of 1/2 inch.at upper sill plate we found openings of 1/2 to 3/4 of an inch between fiber board and sill plate on exterior side.
our home has taken on moisture behind the vynyl siding and most heavily in the voided areas, it has also started growing mold.
thier is no water sheder between the vynyl and fiber board.
has the Manufacture built my home incorrectly and do you have any literature to support incorrect application. We can not find the company that makes this board,
