Routing electric cables through floor joists

When routing electric cables through under-floor old-style joists, the norm is to drill through the joist, as apposed to cut a notch in top of the joist.

What should you do if there is no room to drill the joist, and/or to route the cable after drilling? If cutting into the top of the joist is the only option, what advice would you give?
