Air has to come from somewhere to feed the fire. If all the windows are closed, an open flue (even though partially blocked by a damper) leaves a great inlet for air, especially a hole directly into the space below the fireplace (that's how air works, you know, cold falls down while hot goes up.) Try opening a window in the room with the fireplace burning to see if the smoking backdraft downstairs stops.
