Wood Stove with creasote problem

I have a solid fuel burning appliance Manafactured for Simpson-Sears
Model # c-427-42900
Serial # 5007494

The creasote is leaking from the pipe going through the wall into the chimney.

1. What do I clean this with?

2. How do I stop the problem?
