Washboard Effect began after installation

Oak hardwood installed over concrete slab by securing 3/4" plywood with nails to concrete. Moisture barriers were placed under and over plywood. The hardwood was placed in the house for approximately 2 weeks, however, the plywood was left on a covered porch outside. Immediately after installation popping sounds could be heard daily and it has a washboard effect. The installer sanded one room smooth, but in places it started doing the same thing again. He wants to resand all rooms to smooth the raised edges. Do you think this will resolve the problem and also do you think the problem was most likely caused by leaving the plywood outside.
