concrete shower pan

Hi there, I need some guidance.
I have stall shower. Long story short. A PVC liner was used, and the concrete poured. Unforunately only about 1/2-3/4in was poured. The tiles and the concrete cracked. The tiles have been removed, and current concrete has adhered to the liner. I want to pour an additional 3in on top of the existing 1/2".
1. Should I use a bonding agent between the two slabs?

2. Should I use a fortifier with the new mix?
3. What concrete do you recommend?
4. Should I cover it while it cures, and what material.
5. How long should it cure before being tiled?
