Installing cermaic tile under toliet

I will be installing cermaic tile in my bathroom tomorrow and i need some advice. I have removed the toliet and the flange that holds the toliet down. When I lay down the tile, do I tile right up to the 4" toilet drain and then mount the flange on top of the tile? ( Drilling the flange mounting holes through the tile and into the concrete) OR.. do I install the flange first to the concrete only and tile around the flange? I will also be replacing the wax seal also. I have laid tile before( not a pro) but never in a bathroom. Thanke for your advice.
