Is my willow tree dead?

I have a weeping willow that was planted about 8 to 10 years ago. I recently built a home on the property and last year started to work the yard in preparation for seeding. Last year I noticed that the tree in question did not bloom on all of the branches and it also was very slow in blooming. This year it has not shown any sign of blooming as of yet and I am a bit concerned, as I do not want to be hasty and remove the tree and start over before knowning if it is dead. Some of the things that I can think of that might have contributed to its demise, Before building we tested the old well on the property, at this time bleach was dumped into the well as it had not been used for a number of years. When they blew the well, Pressure tested, the water with the bleach was soaked into the ground around the base of the tree, as the tree is only 25 feet away from the well. Also in working the ground for my future lawn I may have gotten too close to the base of the tree, within a few feet. I worked the lawn with large farm equipment as my father is a farmer who lives down the road from me. Also the last two years have been odd weather wise, we have had hard driven rain or no rain at all. The soil i have is a combination of clay black sand sugarsand and about 2 to 3 inches of black soil.
The tree is about 15 feet tall and was healthy until about a year and half ago.
The tree bark seems to be loosening from the trunk and I can see tiny black bugs under it along with what looks like worm trails, I assume from the insects now calling this tree home. Any information would be great, as I do not want to loose this tree but in the event that it can not be saved I would like to replant a new willow in its place.
