Your contractor installed underlayment with an pneumatic nailer. The problem is that the underlayment should have been installed with underlayment glue and ring-shank nails or screws that do not back out. If this was done by a legitimate contractor, call him back to make the repairs. You may experience frequent recurring problems with this installation, but it is the contractor's fault. Dont do it yourself until you have talked with the contractor. If the contractor was retained by the flooring vendor, its their problem.

If this was a non-contractor install, remove the affected square, and pull the nail. Replace it with a ring shank nail or flush driven screw.
