It's best to remove the old crown molding, if you have it, and to repair and repaint the walls and ceiling. Base molding removal depends on what you want it to look like. You will want to prefinish the new moldings as far as you can before you install them. For painted moldings, you can either install them after priming and a coat of paint, then fill holes and fill gaps with paintable caulk and finally do a final coat of paint or you can paint completely and spot touch-up after installation. For stained and clear finished moldings, you will want to install finished and touch-up.
As an aside, if you are installing big crown moldings, you might consider ripping 2x4's at an angle to take up most of what would be an empty space behind the molding, nailing the ripped 2x4 to the wall at the studs, then nailing the crown to the 2x4. That makes installation easier since you don't have to worry about marking for and hitting a stud while getting the angled crown molding into just the right position.
