Need help on wiring. Wife bought HUNTER fan that was re-wired to a plug since it was used for display.

The fan has only (2) wires, white and black and a 3rd green wire that is attached only to the metal support brace not the fan itself.

The ceiling has black, white, bare and a 4th black with a cap on it. At the wall there is a double switch.

Do I go bare(ceiling)to green(fan)/ white(ceiling) to white(fan) and then both black(ceiling) to black(fan) or just one black from ceiling to fan ??
