Vapor barrier and moisture problem leading to mold

I have just noticed what appears to be mildew/mold on the lower portion of an outside wall in the kids room. Upon closer inspection, i found the lower 1" or so to be damp and the carpeting to be damp as well. I removed the drywall in the damp area and discovered that there was a plastic vapor barrier covering the insulation. The insulation vapor barrier was still in place, but appears to be installed incorrectly. I live in the Gulf Coast area of Texas. How should I repair this mildew problem? Do i need the plastic vapor barrier on the outside wall? Do i need to remove the paper vapor barrier from the insulation? What is causing the condensation on between the plastic and the insullation? I believe the condensation is rolling down the plastic and reaching the plate. Once the plate is saturated, the moisture is reaching the carpet. What would be the best way to repair this situation so that it doesn't return?
