toilet mounting flange-- slab floor

I have a 30 year old single story home in texas (no basement) with lots of previous slap-dash home repair work. Bathroom floor is slab covered with owner installed linoleum tiles. Removed toilet as part of mold renovation/rebuild/tub & surround replacement/wet floor issues. When I lifted the toilet from the drain pipe, the flange came with it. As far as I can tell from examining the floor, the flange was never mounted to the floor (no sign of screw holes in the concrete). HERE'S THE QUESTION --
Is there some kind of installation where the mounting flange attaches to pipe instead? Or does it have to be screwed into the concrete? It's possible that cement has been added on top of the floor around the pipe, or it might just be rough construction. I have a new wax ring and floor bolts, but what else do I need to do/get to reset this toilet properly?
I admit this is my first time looking at the particular piece of toilet anatomy, so I can't tell what's abnormal.
Also, do I need to replace the rubber seal device (somebody called it a spud?) between the tank and toilet?
