in my case, there had been prior repairs. the repaired area had wonderboard cement backer. that was the area where i had the problems. the old, original installation was on greenboard but was fine as it had the black mastic, whereas the repaired area had the cream-colored mastic. the main problem was in the vacinity of the soap dish, which i suspect had been used as a handle when getting up and down in the tub, thus breaking the grout seals. any way, i always use the thinset in the bathroom so that grout leaks will only cause mildew and not failure of the tile.
