This could be happening for a number of reasons.
What are you SPECIFICALLY using for paint?What did you SPECIFICALLY use for primer?Do you know what type of wood it is?Are you using Latex or Oil-based paint?Are you shaking the can to mix the paint or are you stirring it? And for how long?
