sounds like the grout has not been sealed allowing moisture to penetrate to the adhesive. the best installation for a bath or shower is to install cement backerboard and use thinset cement to fasten the tiles to the backer. with thinset and cement board any moisture penetration will not cause the tiles to loosen.
