Ice dams are formed from having too much heat in an attic or rafter space.
The heat thaws snow and ice on the roof, causes it to run down the roof as melt water, then the meltwater refreezes as it approaches the colder eaves.
The cause of ice dams is excess heat in the attic or rafter spaces with poor ventilation being some of the problem.
Presuming the ventilation and even insualtion is normal for the house and climate, then you have to look elsewhere for heat leaks into your attic.
This could come from recessed lights built into the ceiling, chases, pipes, wires, ducts ir any other opening from the living space into the attic space and only by eliminating the source of excess heat can you solve the ice dam problem.
In this case, neither a vapor barrier nor added insulation nor ventilation or a new roof will solve the problem.
