Here's the info from the manufacturer (Chamberlain). The answer to my problem (and probably yours) is at the bottom:

Thank you for your recent e-mail regarding your Garage Door Opener. When programming your remote controls, be sure that you press and immediately release the Smart Button. If you hold the smart button in you may be erasing the code for the previous remote/keyless entry.

The following is a set of instructions for clearing out the receiver and re-programming the remotes:

1) Locate the LEARN BUTTON on the back or side panel of the motor unit. You may need to
remove a light cover or control panel cover to gain access. This button may be green, red or purple
and will have an LED next to it.

2) Push and hold the LEARN BUTTON for six (6) seconds until the LED goes out. This will clear
out all receiver codes.

3) Press and release the LEARN BUTTON. The learn indicator light will glow steadily for
30 seconds.

4) Within this 30 second period, press and hold the button on the hand-held remote.

5) Release the button when the motor unit lights blink. It has learned the code. If light bulbs
are not installed, two clicks will be heard.

6) Repeat steps three & four for each additional remote control/keyless entry.

The following is a set of instructions for re-programming the keypad:

1) Locate the LEARN BUTTON on the back or side panel of the motor unit. You may need to
remove a light cover or control center cover to gain access. This button may be yellow,
white, gray, green red or purple in color and will have an LED next to it.
2) Push and release the LEARN BUTTON. This turns the receiver on for thirty seconds. Within this 30 second period, you must enter your new 4-digit PIN into the keypad and hold the ENTER button for five seconds.

3) When the light on the keypad stops flashing, test your new code by entering the 4-digit PIN
and ENTER.

If these steps are being followed to no avail, then it is safe to assume that one of your remote controls has stopped rolling it's code and has reverted back to a fixed code technology. When a remote control reverts, it no longer has the ability to send a rolling code. When this happens, the receiver will accept either the remote that has reverted, or the remote that is still sending rolling code, but it won't accept both the fixed code and rolling code at the same time. If this is the case you will need to replace the remote control with a new rolling code remote control.
