Level floor

Depending on how bad it is and what you intend to put down as new flooring, I see two courses.

1) Leave it and work with the old house as is. Its character.

2) Leveling compoud and a new layer of sub floor, 3/8 to 1/2 inch if height is concern and the sub floor is solid underneath.
This is most important if the new flooring material your looking to put down requires a level playing field. Us this for tile, linoleum, and laminate flooring.

If height is not an issue, the sub floor is 3/4, 1 by's, then I would put down leveling compound, a new layer of 3/4 tongue and grove plywood and then build the new flooring on that. Use this for hardwood strip flooring.

You can also remove the old floor and sub floor down to the rafters, shim and level with the new tongue and grove, but this is adding much more work.
