Just a guess, but there is water getting under your attempts and freezing which causes the pop outs. You might try drilling sideways in your hole with a masonry bit and installing short pieces of steel rod. I'd also suggest you patch it with a modified concrete, that means it is mixed with latex rather than water. You might also "paint" some latex around inside the hole before you patch it again. Keeping the water out should be the top priority.
