White White/Neutral
Black Black/Switched/Fan
Green Ground
Black w White Switched/Light
If you have one wall switch to control the fan and light, then connect both the black fan wire and the black with white stripe fan wire to the black house wire.
If you have 2 wall switches, one for the fan and one for the light, then connect the black fan wire to the black house wire, and the black with white stripe wire to the house wire from the wall switch controlling the light, which is usually red.
