My experience is that there are not standarized parts that are interchangeable. You may need to just replace the whole extension mechanism, such as
http://www.rockler.com/product.cfm?page=769&filter=table%20hardware

But look around that site and see if they have what you need as parts, since I'm not quite sure what you are looking for.
