There are lots of things that can go wrong in the mechanism -- sheared rivets, bent frames, springs that break or fall off, worn spots, interference, etc.

About all you can do is flip the thing on its back and go to work with a flashlight looking for something that is not working the way it's supposed to.

There are a few of the above things that can be fixed, others cannot and you will have to replace the mechanism.
