you did not say the width of the cracks. I have filled small cracks about 3 feet long and about 1/8 to 1/4 inch in width with vinyl mortar or latex concrete caulk. Both come in colors.
Now, the second part of your question, something is causing this to happen, and I imagine its water causing upheavals or dry causing settling.
