Did you open up the wall at the pipe couplings where the shower head comes through the wall? You have not done any nailing on the wall around or near the shower? I ask this because I found one leak one time and the culprit was a 4Dfinish nail shot into the pipe from the adjoining wall
You have either a pipe fitting leaking or bad caulking around the tub and allowing water to seep behind the tup and into the wall.
