If this duct is for heating only, you can attach aluminum flex duct to the flat duct with a round takeoff after you cut the hole out. Like dryer vent duct it pulls out from a slinky-like compression in the box to a longer piece.
You can slip an insulation sleeve over the duct. Insulated plastic flex duct can slip through as well.
