moldings

i took the moldings off the entranceway from my kitchen to my living room and some of the paint peeled off the wall when i removed the moldings, how do i repair it, i have plaster walls, do i spackle it? and how do i get the surface smooth where i'm going to put the new moldings on, do i sand? what if there is lead paint underneath? if anyone can help with any part of my questions....I WOULD BE GREATFUL!!!
