custom shower pan

I need to find a mamufacturer or insight on a custom shower pan. the dims. are 39 to 40 "W x up to 68"L. i need to to have the waist outlet to be aprox. 36" from the lenghth wall. a custom cut oout would be prefered. in advance thank you!
