If it's the same jack that this problem is happning check the wires in the modular conector box. pop off the cover and check if any of the screws are loose and the connection is good,if they are,then change out the Jack. Go to radio shack or Home depot and get a new Modular wall mounting box and just follow the color code and the Instructions.Any more questions post back.. here.
